Annual TMDL forms submitted by MS4 permittees
If you are assigned a Wasteload Allocation (WLA) in a Total Maximum Daily Load (TMDL) that was approved by the United States Environmental Protection Agency (USEPA) prior to August 1, 2013, and were not meeting that WLA(s) when you applied for permit coverage, you must complete this form to demonstrate progress toward meeting those WLA(s). 2021 TMDL Annual Report Forms are due June 30, 2022.
Warning: The 2021 TMDL Annual Report Form is only requesting information based TMDLS approved prior to the issuance of the 2013 MS4 General Permit. You will begin reporting on TMDLs listed in your 2020 TMDL Application Form in the next reporting period (due June 30, 2023).
