TMDL Application Form

Information: Download your TMDL application form here

If you (an MS4 Permittee) are assigned one or more applicable Wasteload Allocation (WLA) in a Total Maximum Daily Load (TMDL) that was approved by the United States Environmental Protection Agency (USEPA) prior to November 16, 2020, you must download and complete your customized TMDL Application form. See Summary of TMDL requirements in stormwater permits and Guidance for completing the MS4 Permit TMDL Application Form for more information.
