If you are assigned a Wasteload Allocation (WLA) in a Total Maximum Daily Load (TMDL) that was approved by the United States Environmental Protection Agency (USEPA) prior to August 1, 2013, and were not meeting that WLA(s) when you applied for permit coverage, you must complete this form to demonstrate progress toward meeting those WLA(s). Below you will find the 2019 Annual Report you submitted to the MPCA, minus the contact information entered in Cells E24, E27-29 on the User Spreadsheet Tab. Please update those cells, as well as the rest of the tabs with stormwater BMPs installed, and your cumulative pollutant reductions in 2020. Forms are due June 30, 2021.
