If you are assigned a Wasteload Allocation (WLA) in a Total Maximum Daily Load (TMDL) that was approved by the United States Environmental Protection Agency (USEPA) prior to August 1, 2013, and were not meeting that WLA(s) when you applied for permit coverage, you must complete this form to demonstrate progress toward meeting those WLA(s). See Guidance for completing the TMDL reporting form for more information.
