Footnotes: The pdf version of the flow chart requires a user to remember the most restrictive Flexible Treatment Option (FTO) encountered as they proceed through the flow chart. To avoid this issue, the wiki version of the flow chart utilizes additional steps. The wiki version of the flow chart therefore does not match the pdf version.
