If you (an MS4 Permittee) are assigned one or more applicable Wasteload Allocation (WLA) in a Total Maximum Daily Load (TMDL) that was approved by the United States Environmental Protection Agency (USEPA) prior to November 16, 2020, you must download and complete your customized TMDL Application form. Submission of this completed form fulfills the permit requirement for sections 12.8 and 12.10 to either submit a compliance schedule for WLAs not met for oxygen demand, nitrate, total suspended solids (TSS), and total phosphorus (TP); or to provide documentation to demonstrate applicable WLA(s) are being met. See Summary of TMDL requirements in stormwater permits and Guidance for completing the MS4 Permit TMDL Application Form for more information.
Attach your completed TMDL Application Form (and any accompanying supplemental information) to your submittal email for the MS4 Part 2 Application (PDF).
Permittees in the table below have no applicable WLAs to report on in the TMDL Application Form. All other permittees can download their customized TMDL application form HERE.
